SUPPLEMENTAL LIFE AND AD&D INSURANCE
SIMKO provides you with the option to purchase supplemental life insurance for yourself, your spouse, and your dependent children through Cigna. You must purchase supplemental coverage for yourself in order to purchase coverage for your spouse and/or dependents. Supplemental life rates are age-banded (listed on page 11). Benefits will reduce to 50% at age 70.
If you elect coverage when first eligible, you may purchase up to the guarantee issue amount(s) without completing a statement of health (evidence of insurability). If you do not enroll when first eligible, and choose to enroll during a subsequent annual open enrollment period, you will be required to submit evidence of insurability for any amount of coverage. Coverage will not take effect until approved by Cigna.
- Employee: $10,000 increments up to the lesser of $500,000 or 5x annual salary Guarantee issue: Lesser of $150,000 or 5x annual salary
- Spouse: $5,000 increments up to the lesser of $250,000 or 50% of the employee election Guarantee issue: $30,000
- Dependent children: Birth to 6 months: $500; 6 months to age 26: $2,500 increments up to $10,000 Guarantee issue: $10,000
Listed below are the monthly rates for supplemental life insurance. The amount you pay for supplemental life insurance is deducted from your paycheck on a post-tax basis.
DISABILITY INSURANCE
Voluntary Short Term Disability Insurance
SIMKO provides you with the option to purchase voluntary short term disability insurance (voluntary STD) for yourself. Voluntary STD rates are age-banded (listed on page 11). Voluntary STD insurance is designed to help you meet your financial needs if you become unable to work due to an illness or injury. Benefits will be reduced by other income, including state-mandated short term disability plans.
Benefit: 60% of base weekly pay up to $1,000
- Elimination period: 7 days
- Benefit duration: Up to 12 weeks
- Pre-existing condition limitation: 6 month lookback period or 12 month waiting period. Benefits will not be paid for any disability for which you received medical treatment, care, or consultation for during the 6 months preceding your effective date until you have been covered under the policy for 12 months
*The pre-existing condition limitation will not apply to employees who were previously covered under the Smiles in Motion or Kristo Orthodontic short-term disability program for 12 months preceding coverage under the Cigna plan. Credit for time covered under the previous plan will be given.

ACCIDENT INSURANCE
Voluntary Accident Insurance—Cigna
SIMKO provides you the option to purchase accident insurance through Cigna.
Accident insurance helps protect against the financial burden that accident-related costs can create. This means that you will have added financial resources to help with expenses incurred due to an injury, to help with ongoing living expenses, or to help with any purpose you choose. Claims payments are made in flat amounts based on services incurred during an accident.

CRITICAL ILLNESS INSURANCE
Voluntary Critical Illness Insurance—Cigna
SIMKO provides you the option to purchase critical illness insurance through Cigna.
Critical illness insurance provides a financial, lump-sum benefit upon diagnosis of a covered illness. These covered illnesses are typically very severe and likely to render the affected person incapable of working. Because of the financial strain these illnesses can place on individuals and families, critical illness insurance is designed to help you pay your mortgage, seek experimental treatment, or handle unexpected medical expenses.
Employee: $5,000; $10,000; or $20,000. Guarantee issue: $20,000
Spouse: 50% of the employee election. Guarantee issue: $10,000
Dependent children: 50% of the employee election. Guarantee issue: $10,000
Health screening benefit: $50 per year
