BASIC LIFE AND AD&D INSURANCE
Southwest Human Development automatically provides basic life and AD&D insurance through Unum to all benefits eligible employees AT NO COST. If you die as a result of an accident, your beneficiary would receive both the life benefit and the AD&D benefit. Please be sure to keep your beneficiary designations up to date.
- Employee life benefit: 1x annual earnings up to a maximum of $250,000
- Employee AD&D benefit: 1x annual earnings up to a maximum of $250,000
SHORT-TERM DISABILITY INSURANCE
Southwest Human Development automatically provides short-term disability (STD) insurance through Unum to all benefits-eligible employees AT NO COST. STD insurance is designed to help you meet your financial needs if you become unable to work due to an illness or injury. Benefits will be reduced by other income, including state-mandated STD plans.
- Benefit for less than 2 years of service: 60% of base weekly pay
- Benefit for 2 years to less than 5 years of service: 70% of base weekly pay
- Benefit for 5 years or more of service: 100% of base weekly pay for 2 weeks; 80% of base weekly pay for 2 weeks; 70% of base weekly pay for remaining period
- Weekly benefit maximum: $2,200 per week
- Elimination period: 7 days for sickness/injury
- Benefit duration: 25 weeks
- Pre-existing condition exclusion: None
