P.F. Chang’s offers two dental insurance plan options through Aetna.
The plans offer in- and out-of-network benefits, providing you the freedom to choose any provider. However, you will pay less out of your pocket when you choose an Aetna provider. Locate an in-network provider at aetna.com.
The table below summarizes key features of the dental plans. The coinsurance amounts listed reflect the amount you pay. Please refer to the official plan documents for additional information on coverage and exclusions.
1) Out-of-network fees are based on usual and customary charges. When you go out-of-network, you are responsible for the balance over the reasonable and customary charges.

VISION BENEFITS

The plans offer in- and out-of-network benefits, providing you the freedom to choose any provider. However, you will pay less out of your pocket when you choose an EyeMed provider. Locate an in-network provider at eyemed.com.
The table below summarizes key features of the vision plans. Please refer to the official plan documents for additional information on coverage and exclusions.
1) Plan allows the member to receive either contacts and frame or frame and lens services in the same calendar year.
2) Plan allows the member to receive contacts, frame, and lens services in the same calendar year.
Note: Retinal imaging covered by both plans at $0 copay.
