The Kenan Advantage Group provides you the option to purchase voluntary long-term disability (LTD) insurance through Unum. LTD insurance is designed to help you meet your financial needs if your disability extends beyond the short-term disability period.

The amount you receive from this policy may be lower if you also receive funds from other sources for your disability such as social security disability insurance from the government. Class 1—All Administrative and Salaried Employees of KAG West AZ, NV, and CA, and All Administrative and Salaried Employees of Klemm:

• Benefit: $100 increments up to $4,000 per month, not to exceed 60% of monthly earnings
• Elimination period: 90 days
• Benefit duration: To age 65 or longer depending on your age when you become disabled

Class 2—Drivers, Mechanics/Technicians, and Tank Washers:

• Benefit: $100 increments up to $2,200 per month
• Elimination period: 90 days
• Benefit duration: To age 65 or longer depending on your age when you become disabled

Pre-Existing Condition Limitation

This plan does not cover pre-existing conditions if the disability begins in the first 24 months after your effective date of coverage (unless the covered individual has remained treatment free for 12 consecutive months beginning on or after the effective date of coverage). You have a pre-existing condition if you received medical treatment, consultation, care, or services (including diagnostic measures) for the condition, or took prescribed medications for it in the 12 months prior to your effective date of coverage.
