Blue Origin offers two dental insurance plan options through Delta Dental of Washington (WA).

The plans offer in- and out-of-network benefits, providing you the freedom to choose any provider. However, you will
pay less out of your pocket when you choose a Delta Dental WA provider. Locate a Delta Dental WA network provider at deltadentalwa.com. Register online to get your ID card, view your benefits, download forms, or compare treatment costs. ID cards will show the employee's name only, even when dependents are covered on the plan.

The table below summarizes key features of the dental plans. The coinsurance amounts listed reflect the amount you pay. Please refer to the official plan documents for additional information on coverage and exclusions.

DENTAL COSTS

Listed below are the per pay period costs for dental insurance. The amount you pay for coverage is deducted from your paycheck on a pre-tax basis, which means you don’t pay taxes on the amount you pay for coverage. The premium for your domestic partner (and their children) will be paid on a post-tax basis and may result in imputed income to you.

    ENROLLMENT INSTRUCTIONS

    How To Enroll:

    NEW HIRES

    1. Complete the Change Benefits for Life Event enrollment task in your Workday inbox.
    2. Click Let's Get Started to waive or elect each of the benefits offered to you.
    3. Elect your plans, add your family members, then click Review and Sign.
    4. Review your benefits elections.
    5. Upload any required documentation, read and accept, and click Submit.
    6. Download and save your Benefits Statement.

    MID YEAR CHANGES

    1. Click the Benefits Application to submit your benefit changes
    2. Elect your plans, add your family members, then click Review and Sign.
    3. Review your benefits elections.
    4. Upload any required documentation, read and accept, and click Submit.
    5. Download and save your Benefits Statement.
